About the Job:
The Licensed Practical Nurse - Transition Liaison plays a vital role in ensuring seamless patient care transitions between Interior Health facilities and healthcare services.

Key Responsibilities:

• Collaborate with multidisciplinary teams to plan, implement, and evaluate patient care transitions.

• Act as a resource for patients, families, and healthcare teams to assess patient needs and eligibility for services.

• Operate within the scope of practice as outlined by the British Columbia College of Nurses and Midwives, and according to Interior Health policies and protocols.

This is a casual opportunity with Interior Health, offering flexibility and variety in scheduling. As a casual employee, you'll have the opportunity to apply for internal postings, including permanent and temporary positions.

Shifts will be scheduled during the day, rotating between 08:00 and 16:00 hours.
