Job Summary:

The Transition Liaison Registered Nurse plays a vital role in providing high-quality patient care and ensuring a seamless transition between healthcare services. As a key member of our interdisciplinary team, you will work closely with patients, families, and healthcare professionals to assess patient needs, develop care plans, and coordinate access to necessary services.

Key Responsibilities:

  • Provide comprehensive nursing care to patients, working independently and collaboratively with other healthcare professionals
  • Enhance patient quality of life by identifying and addressing variances in care plans and service needs
  • Develop and implement effective care transition plans, ensuring patients receive necessary services and support
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination
  • Stay up-to-date with best practices and evidence-based care guidelines to continuously improve patient outcomes
