About the Role
This is an opportunity to work in Long Term Care, where you will be part of a multidisciplinary team responsible for assessing and treating residents. Your duties will include collaborating with healthcare professionals to develop effective care plans, leading team members to achieve treatment goals, and providing education to residents and their families.
Key Responsibilities
- Collaborate with a team of healthcare professionals to assess and treat residents
- Develop and lead care plans to prevent deterioration and manage pain
- Provide education and support to residents, their families, and other caregivers
- Demonstrate flexibility in scheduling to accommodate home life needs
