Job Summary

Fraser Health is seeking a skilled Registered Nurse/Registered Psychiatric Nurse to join our team. As a key member of our interdisciplinary team, you will provide person-centered care to diverse populations, promoting resident/client choices, individualized care plans, and supporting continuity and consistency of care.

Responsibilities
  1. Collaborate with the interdisciplinary team to identify current or potential care concerns and strengths, plan interventions, and evaluate outcomes.
  2. Provide direct nursing care, including wound care, dementia care, and palliative care, and develop care priorities in accordance with established policies and standards of nursing practice.
  3. Collect information from various sources using skills of observation, communication, and physical assessment to enable the provision of quality care.
  4. Document relevant observations, assessments, and interventions, including resident/client and family teaching and evaluation of care.
  5. Communicate resident/client assessments and changes to Physicians, Resident/Client Care Coordinator, and the interdisciplinary team.
  6. Act as an advocate to protect and promote residents'/client's rights to autonomy, respect, privacy, dignity, diversity, and access to information.
  7. Work in partnership with Licensed Practical Nurses and other interdisciplinary team members by sharing information, demonstrating, and/or providing guidance on clinical tasks.
  8. Participate in quality improvement activities by gathering information, providing input into proposed policies and revisions of current policies.
  9. Maintain records, prepare reports, and collect statistical and quality improvement data for research and evaluation purposes.
  10. Maintain a safe working environment by following health and safety guidelines and reporting unsafe, hazardous conditions.
Qualifications

Education and Experience:

  • Graduate of an approved School of Nursing or Psychiatric Nursing.
  • Current practicing registration as a Registered Nurse and/or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives.
  • One (1) year's recent, related gerontological nursing experience or an equivalent combination of education, training, and experience.

Skills and Abilities:

  • Knowledge of nursing theory and evidence-informed practice.
  • Knowledge of health care disciplines and their roles in health care.
  • Ability to apply clinical knowledge in practice.
  • Ability to apply critical thinking skills, problem solve, and effectively resolve conflict situations.
  • Ability to communicate effectively both verbally and in writing.
  • Ability to provide leadership and clinical guidance.
  • Ability to work independently and as a member of the interdisciplinary team.
  • Ability to plan, organize, and prioritize work in a self-directed manner.
