Nurse Practitioner

The Registered Nurse position at Fraser Health Authority offers a unique opportunity to work in a dynamic and growing organization, collaborating with a team of healthcare professionals to provide exceptional patient care. As a Registered Nurse, you will be responsible for assessing, planning, and delivering nursing care to patients in acute inpatient areas, working in partnership with the interprofessional care team to provide person-centered care.
Responsibilities- Conduct thorough patient assessments, collecting information from various sources and making informed nursing diagnoses to determine the best course of care.
- Develop and implement individualized care plans, collaborating with patients and their families/caregivers to ensure their needs are met and their preferences are respected.
- Collaborate with the interprofessional care team to ensure comprehensive care planning and delivery, participating in care conferences and councils as assigned.
- Provide education and support to patients and families/caregivers, promoting health literacy and empowering them to take an active role in their care.
- Document patient information and care plans accurately and efficiently, using relevant computer applications and adhering to practice standards and Fraser Health procedures.
- Participate in quality improvement activities, identifying areas for improvement and contributing to the development of evidence-based practices.
- Act as a preceptor or mentor to new nursing staff and students, sharing knowledge and expertise to support their growth and development.
To be successful in this role, you will require:
- Graduation from an approved School of Nursing with current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families/caregivers, and healthcare teams.
- Strong clinical skills and knowledge, with the ability to assess and manage complex patient needs.
- Ability to work in a fast-paced environment, prioritizing tasks and managing multiple responsibilities with ease.
- Physical ability to perform the duties of the position, with the ability to lift, move, and position patients as required.
