Job Overview
We are seeking a highly skilled Home Care Nursing Professional to join our team at Vancouver Coastal Health. As a Community Health Nurse, you will provide client-focused care to individuals with acute, chronic, and palliative health conditions in the comfort of their own homes or community settings. You will work in partnership with clients, families, and other healthcare professionals to establish goals, develop care plans, and deliver direct care to assist clients in managing their own care and navigating through various services available within the community.

Salary
The salary range for this position is approximately $41,420 - $55,910 per year, depending on experience and qualifications. This is a competitive salary that reflects the high standards of care we expect from our nursing professionals. Responsibilities
Your primary responsibilities as a Home Care Nursing Professional will include:
- Providing high-quality, client-centered care to individuals with complex health needs
- Assessing clients' needs, developing care plans, and implementing interventions to promote health and well-being
- Working collaboratively with clients, families, and healthcare teams to achieve optimal outcomes
- Managing caseloads, prioritizing tasks, and ensuring timely completion of care activities
- Maintaining accurate records, documenting assessments, interventions, and outcomes in electronic medical records (EMRs)

Qualifications
To be successful in this role, you must have:
- Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM)
- One year recent related clinical experience in a relevant clinical area working with complex clients/patients with chronic health conditions and/or functional mobility issues
- Valid BC Driver's License required as local area travel may require the use of a vehicle or other accepted mode of transportation

Requirements

- Demonstrated knowledge and skills in community health nursing theory and practice within a client- and family-centred model of care
- Assessment and intervention skills, including trauma-informed and culturally safe care
- Critical thinking and problem-solving abilities, with attention to safety and risk assessment
- Excellent communication and interpersonal skills, with ability to work collaboratively with clients, families, and healthcare teams
- Ability to prioritize tasks, manage multiple priorities, and maintain accurate records
