About the Role

We are seeking a skilled and compassionate individual to provide home support services to clients in accordance with established care plans. As a Community Health Worker, you will work collaboratively with an interprofessional healthcare team to promote independent living, health, and wellbeing among our community members.

Job Responsibilities
  • Provide personalized care and support to clients, prioritizing their physical and emotional needs.
  • Assist with daily activities such as bathing, grooming, and meal preparation, promoting maximum independence.
  • Collaborate with healthcare professionals to develop and implement care plans tailored to individual client needs.
  • Maintain accurate records and communicate effectively with clients, family members, and healthcare teams.
Requirements and Qualifications

To be successful in this role, you will require:

  • A recognized Home Support Attendant/Resident Care Aid Program diploma or equivalent experience.
  • Registration with the BC Care Aid & Community Health Worker Registry.
  • A valid Class 5 BC Driver's License.
  • Certificates in CPR, First Aid, and Food Safe.
  • The ability to establish and maintain rapport with clients, observe and recognize changes in their condition, and communicate effectively with diverse groups.

Location and Salary

This position is based in the Upper Skeena region, covering communities such as Hazelton, New Hazelton, and surrounding areas. The estimated annual salary for this role is $58,000 - $65,000, depending on experience and qualifications.
