Job Title: Social Worker II - Primary Care

We are seeking a highly skilled and compassionate Social Worker II to join our Primary Care team at Northern Health Authority. As a key member of our interprofessional team, you will provide comprehensive assessment, care planning, and intervention for individuals and families in our community.

Job Summary:

The Social Worker II will work collaboratively with healthcare providers and the interprofessional team to provide direct psychosocial care, counseling, and group work, as well as system navigation and advocacy. You will be responsible for providing social work services to maintain or improve the health and functioning of persons, families, and communities.

Responsibilities:
  • Provide comprehensive assessment and care planning for individuals and families
  • Develop and implement individualized care plans to address social determinants of health
  • Provide direct psychosocial care, counseling, and group work to individuals and families
  • Collaborate with healthcare providers and the interprofessional team to provide seamless care
  • Advocate for individuals and families to access community resources and services
Requirements:
  • Bachelor's Degree in Social Work from an accredited university
  • Two years recent related experience in a healthcare setting or a combination of related education, training, and experience
  • Current registration with the British Columbia College of Social Workers
  • Current valid BC driver's license
