Nurse Practitioner

Job Summary:
We are seeking a dedicated and compassionate Registered Practical Nurse to join our team at Southbridge Care Homes. As a key member of our healthcare team, you will play a vital role in providing exceptional care to our residents.
Key Responsibilities:
- Deliver High-Quality Care: Provide individualized care to residents, ensuring their physical, emotional, and social needs are met.
- Collaborate with Team Members: Work closely with other healthcare professionals to develop and implement care plans that meet the unique needs of each resident.
- Monitor and Document Resident Care: Regularly assess and document resident vital signs, symptoms, and conditions, and take appropriate action to address any concerns.
- Lead and Train Team Members: Supervise and train new staff members, providing guidance and support to ensure they are equipped to provide high-quality care.
- Participate in Quality Improvement Initiatives: Contribute to quality improvement initiatives and participate in ongoing education and training to stay up-to-date on best practices in long-term care.
Requirements:
- Certification and Registration: Current certificate of competence from the College of Nurses of Ontario and registration with the Registered Practical Nurses Association of Ontario.
- Language Skills: Bilingual in English and French.
- Experience and Skills: Previous experience in long-term care, with a strong understanding of duties and responsibilities. Excellent communication and interpersonal skills, with the ability to work effectively in a team environment.
