Job Summary
We are seeking a skilled Community Health Transition Specialist to join our team at PHSA. As a Community Health Transition Specialist, you will play a critical role in supporting clients with complex mental health and substance use challenges as they transition from correctional facilities to community-based care.
Key Responsibilities
- Develop and implement transition plans for clients, ensuring seamless continuity of care and support.
- Collaborate with interdisciplinary teams, including healthcare professionals, social workers, and community agencies, to provide comprehensive support services.
- Conduct assessments and identify client needs, developing individualized plans to address these needs.
- Provide education and support to clients and their families, promoting healthy behaviors and self-management skills.
- Facilitate connections to community resources and services, ensuring clients have access to necessary support and care.
Requirements
- Graduation from an approved School of Nursing with current practicing registration as a Registered Nurse or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
- Two (2) years recent, related experience in acute or primary care or community health nursing or an equivalent combination of education and experience.
- Successful completion of Occupational First Aid II (OFA II) (or OFA III techniques for positions at Ford Mountain Correctional Centre).
