About the Role

We are seeking a skilled Registered Nurse to join our team as a Nurse Care Coordinator at Fraser Health Authority. As a key member of our healthcare team, you will play a vital role in providing high-quality patient care and ensuring seamless transitions between healthcare services.

Key Responsibilities
  • Assess patient needs and develop comprehensive care plans in collaboration with the interprofessional team.
  • Collaborate with patients, families, and healthcare providers to ensure effective communication and coordination of care.
  • Provide education and support to patients and families to promote self-management and independence.
  • Conduct thorough assessments and identify potential health risks to develop targeted interventions.
  • Work closely with the healthcare team to ensure timely and efficient care transitions.
  • Participate in quality improvement initiatives to enhance patient outcomes and experiences.
Requirements
  • Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
  • Strong assessment and communication skills, with the ability to work effectively in a fast-paced environment.
  • Experience in care coordination and patient education is an asset.
  • Ability to work collaboratively as part of a multidisciplinary team.
  • Strong problem-solving and critical thinking skills.
