Key Responsibilities- Assessment and Care Planning: Conduct thorough assessments of patients to identify their physical and psychological needs, and develop and implement individualized care plans to address these needs.
- Collaboration and Communication: Work collaboratively with patients, families, and the interprofessional team to ensure seamless care coordination and effective communication.
- Education and Support: Provide education and support to patients and their families to promote health and wellness, and empower them to take an active role in their care.
- Documentation and Reporting: Maintain accurate and comprehensive patient records, and report significant changes or findings to the physician or other healthcare providers as required.
- Quality Improvement: Participate in quality improvement initiatives to identify areas for improvement and implement changes to enhance patient care and outcomes.
- Graduation from an approved school of nursing and current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
- Ability to assess patients and apply assessments to determine appropriate patient care interventions.
- Ability to develop and implement a comprehensive plan of care.
- Knowledge of chronic management models and ability to teach and promote chronic disease self-management.
- Ability to communicate effectively, both verbally and in writing.
- Ability to organize and prioritize work in a continuously changing environment.
- Ability to work independently and collaboratively as a member of an interprofessional care team.
