We are seeking a highly skilled and experienced Registered Nurse to join our team at Fraser Health. As a Registered Nurse - Patient Care Specialist, you will play a critical role in providing high-quality patient care and contributing to the development of our clinical programs.
Key Responsibilities- Conduct comprehensive nursing assessments of patients and develop individualized care plans to meet their unique needs.
- Collaborate with interdisciplinary teams to coordinate patient care and ensure seamless transitions between healthcare settings.
- Provide education and support to patients and their families to promote health literacy and self-management skills.
- Participate in quality improvement initiatives and contribute to the development of evidence-based practices.
- Act as a preceptor and mentor to new staff members and students, promoting a culture of excellence and continuous learning.
- Graduation from an approved School of Nursing with current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
- Two years of recent related clinical experience in the designated clinical area, and completion of post-graduate coursework and/or certification applicable to the designated clinical area or an equivalent combination of education, training, and experience.
- Valid BC Driver's License and access to a personal vehicle for business-related purposes as required.
- Clinical proficiency in the designated clinical area.
- Ability to communicate effectively with patients, families, and healthcare teams using verbal, written, and electronic communication means.
- Ability to promote person-centered care that demonstrates empathy, sensitivity to diverse cultures and preferences, and patient advocacy.
