Position Summary

As a Social Worker in our MASTRS & OASIS Programs, you will be an integral part of a collaborative, inter-disciplinary team focused on delivering seamless services and programs to our older adult clients and customers. Key Responsibilities
  • Conduct comprehensive assessments for clients, integrating biopsychosocial history, diagnosis, and treatment planning.
  • Develop and evaluate treatment plans in collaboration with the clinical team, community supports, and families.
  • Facilitate group counselling sessions and provide individual counselling to support patients' specific needs and goals.
  • Ensure continuity of care through the transfer of information and work with the clinical team to initiate community treatment orders.
  • Establish relationships with key community supports and prepare referrals to appropriate community services.
  • Document in a timely manner and collaborate with the multidisciplinary team.
Requirements
  • Master's Degree in Social Work (MSW) required.
  • Registered member of OCSWSSW.
  • Working knowledge of the Mental Health Act.
  • Demonstrated experience working with addictions and concurrent psychiatric disorders.
  • Trained in therapy interventions used in mental health (CBT, DBT, Motivational Interviewing).
  • Knowledge of standardized and non-standardized assessments.
  • Experience in program development and delivery.
  • Knowledge of local community social services.
