Social Worker, MSTRS Program Specialist

Job Summary

We are seeking a skilled Social Worker to join our MASTRS & OASIS Programs team. As a key member of our interdisciplinary team, you will work closely with clients, families, and community partners to deliver seamless services and programs to older adults.

Key Responsibilities
  • Conduct comprehensive assessments to inform treatment plans
  • Develop and evaluate treatment plans in collaboration with the clinical team
  • Facilitate group and individual counseling sessions
  • Ensure continuity of care through effective communication and documentation
  • Work with the clinical team to initiate community treatment orders and establish relationships with key community supports
Requirements
  • Master's Degree in Social Work (MSW) required
  • Registered member of OCSWSSW
  • Working knowledge of the Mental Health Act
  • Demonstrated experience working with addictions and concurrent psychiatric disorders
  • Trained in therapy interventions used in mental health (CBT, DBT, Motivational Interviewing)
