Job Summary:

We are seeking a highly skilled and compassionate Care Coordinator to join our team at Home and Community Care Support Services. As a Care Coordinator, you will play a vital role in managing a caseload of diverse patients, assessing their care needs, and developing personalized care plans that cater to their unique health and psychosocial requirements.

Key Responsibilities:

  • Conduct comprehensive patient assessments to identify clinical, physical, and psychosocial needs, as well as caregiver needs.
  • Collaborate with patients, caregivers, and healthcare teams to develop and implement individualized care plans that address patient priorities and desired outcomes.
  • Coordinate the delivery of Home and Community Care services, including medical equipment, supplies, and services.
  • Facilitate communication among healthcare team members to ensure seamless care coordination.
  • Provide regular reports to physicians, service providers, patients, and caregivers as required.

Requirements:

  • Registered Nurse (BScN or diploma) or equivalent degree in a related field.
  • Related professional experience, preferably in a community-based setting.
  • Knowledge of medical diagnoses, symptom management, and cognitive/psychiatric disorders.
  • Excellent communication, interpersonal, and problem-solving skills.
