Job Title: Care Coordinator

Job Summary:

We are seeking a highly skilled Care Coordinator to join our team at Home and Community Care Support Services. As a Care Coordinator, you will play a vital role in assessing client needs, developing care plans, and coordinating services to ensure the best possible outcomes for our clients.

Key Responsibilities:
  • Client Assessment: Conduct thorough assessments of clients to identify their needs and develop personalized care plans.
  • Care Planning: Collaborate with clients, families, and healthcare professionals to develop and implement care plans that meet the client's needs and goals.
  • Service Coordination: Coordinate services and resources to support clients in their homes and communities.
  • Communication: Maintain effective communication with clients, families, and healthcare professionals to ensure seamless care coordination.
Requirements:
  • Education: Registered Nurse (BScN or diploma) or equivalent degree in a related field.
  • Experience: At least 2 years of experience in a related field, preferably in home and community care.
  • Skills: Strong assessment and care planning skills, excellent communication and interpersonal skills, and ability to work independently and as part of a team.
