Job Summary

We are seeking a highly skilled and compassionate Care Coordinator to join our team at Home and Community Care Support Services Champlain. As a key member of our healthcare team, you will play a vital role in ensuring the delivery of high-quality care to our clients.

Key Responsibilities
  • Assess client needs and develop personalized care plans in collaboration with healthcare professionals and family members.
  • Coordinate and implement care services, including home care and placement, to meet client needs and preferences.
  • Provide education and support to clients and their families on healthcare options and community resources.
  • Collaborate with healthcare teams to ensure seamless transitions and continuity of care.
  • Monitor and evaluate client progress, making adjustments to care plans as needed.
Requirements
  • Registered Nurse (BScN or diploma) or equivalent healthcare professional degree.
  • Minimum 2 years of related professional experience.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Proficient in the use of computerized processes and mobile technology.
What We Offer

We offer a competitive salary range of $40.409/hr - $43.169/hr, Monday to Friday, 8:30am to 4:30pm, at our Hawkesbury Hospital location. We are committed to providing a safe and inclusive work environment and offer opportunities for professional growth and development.
