Job Summary

Vancouver Coastal Health is seeking an experienced Registered Nurse to join the team in the Hospice Program in the Vancouver Community, Vancouver, BC. As a Palliative Care Coordinator, you will play a vital role in administering the palliative care registry and providing administrative support to clients with palliative care needs.

Key Responsibilities

  • Administer the palliative care registry and provide administrative support to clients with palliative care needs.
  • Facilitate regular discussions with clinicians to determine the most appropriate level of care for clients.
  • Determine priority placement and access of clients to community hospice and acute palliative care beds.
  • Participate in the planning and development of an integrated palliative care program.
  • Conduct public education sessions and supervise clerical support staff.
  • Ensure timely generation of required reports and develop tools for data collection.

Qualifications

Education & Experience

  • Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
  • Two (2) years' recent, related nursing experience, including one year's related administrative and palliative care program development experience.
  • Advanced palliative care nursing certification (CHPCN(C)).

Knowledge, Skills & Abilities

  • Knowledge of clinical practice related to care management, interventions, and care of designated client populations.
  • Knowledge and demonstrated skill in the provision of evidence-based clinical practice.
  • Knowledge of other health care disciplines and their role in patient care.
  • Ability to supervise, provide leadership, and work collaboratively as a member of an interdisciplinary team.
  • Ability to communicate orally and in writing, and deal effectively with co-workers, physicians, and patients and their families.
