Salary InformationThe estimated annual salary for this position is $44,742 - $53,976 per year, based on a hourly rate of CAD $42.27 - $52.81.
Job DescriptionThis role focuses on recovery and rehabilitation, reducing barriers to re-enter the system of care, and supports individuals and families with acute/episodic mental illness and substance use disorders. The successful candidate will work as a member of a multi-disciplinary team in community and hospital settings.
- Develops and implements recovery-oriented treatment options and plans in collaboration with clients and natural supports;
- Provides individual, group, and family support using various interviewing, supportive counseling, and assessment methods;
- Supports the intake process, assessing client needs, and determining priority of care and type of level of care required;
- Facilitates client utilization of appropriate community services and rehabilitative programs;
- Advocates on behalf of clients, providing information about appeal processes and completing necessary forms;
- Makes necessary arrangements for clients to be admitted to hospital when required;
- Provides client care linkages between community mental health & substance use services and acute care facilities;
- Maintains client records by recording information such as client assessments, charting client information via PARIS and/or other clinical information systems;
To be successful in this role, you will require:
- Bachelor's degree in Allied Health Discipline from a recognized program;
- Two years' recent, related experience working with clients and families with, or affected by, mental illness and substance use disorders, including one year of experience working with Indigenous clients, or an equivalent combination of education, training, and experience;
- Knowledge of Indigenous health approaches, protocol, and cultural issues;
