Job Summary

We are seeking a skilled Rehabilitation Specialist - Patient Care Coordinator to join our team at Fraser Health. In this role, you will work closely with our therapists to implement programs that meet the needs of residents/patients in Residential Care.

About the Role

This is a unique opportunity to make a meaningful impact in the lives of others. As a Rehabilitation Assistant, you will be responsible for:

  • Assisting therapists in organizing, preparing, and implementing therapeutic programs for assigned residents/patients;
  • Participating in resident/patient activities of daily living by demonstrating and assisting with self-care and meal-management;
  • Assisting with resident/patient mobility, including provision of appropriate wheelchair seating and transfer to/from bed and chair;
  • Reporting relevant observations concerning resident/patient progress and condition to other care team members;
  • Maintaining an inventory of equipment, supplies, and wheelchairs on loan to patients;
What We're Looking For

To be successful in this role, you will require:

  • A completion of a Rehabilitation Assistant Program or an equivalent combination of education, training, and experience;
  • The ability to communicate effectively both verbally and in writing;
  • The ability to deal with others effectively;
  • Physical ability to carry out the duties of the position;
  • The ability to organize work and operate related equipment;
