Our hospice is located in the heart of downtown Toronto, with 13 beds and an international teaching and learning program. We're innovating care options for people in need across Canada and beyond by providing specialized trauma-informed, culturally safe, and harm-reducing approaches to care.

Job Description

You'll work closely with our interdisciplinary healthcare team, including PSWs, RPNs, physicians, nurses, and volunteers, to provide high-quality patient care. Your responsibilities will include:

  • Providing individualized care to patients with complex needs
  • Collaborating with the healthcare team to develop and implement personalized care plans
  • Participating in ongoing quality improvement initiatives to enhance patient outcomes
Required Skills and Qualifications

To be successful in this role, you'll need:

  • A current Certificate of Competence with the College of Nurses of Ontario as a Registered Nurse (RN)
  • LEAP Certificate Required / Willingness to complete LEAP Certificate within 6 months of hire
  • Minimum 1 year Hospice Palliative care experience and ongoing continuing education preferred
  • Experience working with vulnerable populations
  • Mental Health Nursing Certification or equivalent training preferred
  • 1 year Mental Health experience and ongoing continuing education an asset
  • Well-developed interpersonal skills and excellent organizational skills
  • Able to work independently as well as part of an integrated health care team
