Job Title: Social Worker I

Job Summary:

We are seeking a highly skilled and compassionate Social Worker I to join our team at Providence Health Care. As a key member of our interdisciplinary team, you will provide therapeutic clinical counselling services to patients/clients, conduct comprehensive social work psychosocial assessments, and develop treatment options with patient/client input.

Key Responsibilities:
  • Provides therapeutic clinical counselling services to patients/clients, including exploring their present situation, identifying goals, problems, personal concerns, and treatment objectives.
  • Conducts comprehensive social work psychosocial assessments using methods such as interviewing the patient/client, obtaining relevant information, gathering social data, and interpreting the data to therapeutically impact the patient/client's coping abilities.
  • Assesses patient/client needs and available services, develops treatment options with patient/client input, and facilitates patient/clients' utilization of community services and rehabilitative programs.
  • Provides referrals to appropriate community treatment alternatives and services, including recovery and residential programs, alcohol and drug treatment, victim assistance, employee assistance programs, transition homes, vocational programs, and other agencies.
  • Maintains patient/client records through electronic methods, documenting assessments, charting information, preparing progress notes, and reports in accordance with established standards, policies, and procedures.
Requirements:
  • Master's Degree in Social Work from an accredited university.
  • Two (2) years' recent, related experience working with diverse marginalized populations in the areas of HIV/AIDS, substance use/addictions, and/or mental health.
  • Current full registration with the British Columbia College of Social Workers.
