Key Responsibilities- Conduct comprehensive social and psychosocial assessments to identify client needs and develop effective intervention plans.
- Provide individual, family, and group counseling services to promote client well-being and resilience.
- Maintain accurate and up-to-date client records, and participate in interdisciplinary clinical and case conferences.
- Collaborate with community resources and healthcare teams to ensure seamless care coordination and continuum.
- Current full registration with the British Columbia College of Social Workers (BCCSW).
- Master of Social Work from an accredited School of Social Work, and one year of post-Bachelor of Social Work experience in a healthcare setting.
- Broad knowledge of systems thinking, health disciplines, research methodology, counseling skills, and clinical practice models.
- Demonstrated ability to apply systems theory, conduct social and psychosocial assessments, and establish rapport with clients and families.
