Palliative Care Specialist

We are seeking a highly skilled Palliative Care Specialist to join our team at SE Health. With a strong commitment to delivering exceptional patient care, you will play a vital role in providing compassionate and comprehensive support to patients experiencing homelessness and structural vulnerability.
About the RoleThis is an exciting opportunity to work with a diverse range of clients and contribute to the development of innovative approaches to care. As a Palliative Care Specialist, you will be responsible for providing high-quality nursing care to patients in their homes or in our specialized hospices. You will work collaboratively with other healthcare professionals to deliver holistic care that addresses the physical, emotional, and social needs of our patients.
Key Responsibilities- Provide expert palliative care to patients in their homes or in our hospices
- Conduct thorough assessments of patient needs and develop individualized care plans
- Collaborate with interdisciplinary teams to ensure seamless care coordination
- Participate in quality improvement initiatives to enhance patient outcomes and experience
To be successful in this role, you will require:
- A current Certificate of Competence with the College of Nurses of Ontario as a Registered Nurse (RN)
- LEAP Certificate Required / Willingness to complete LEAP Certificate within 6 months of hire
- Minimum 1 year Hospice Palliative care experience and ongoing continuing education
- Experience working with vulnerable populations
- Mental Health Nursing Certification or equivalent training preferred
