We are seeking a highly skilled Senior Care Coordinator to join our team at Reconnect Health Services.

Job Summary:

The Senior Care Coordinator will work closely with clients, family members, and healthcare providers to assess needs and develop personalized care plans.

This role requires strong communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment.

Responsibilities:
  • Conduct comprehensive assessments of client needs and develop tailored care plans
  • Collaborate with healthcare providers and community agencies to ensure seamless service delivery
  • Provide support and guidance to clients and their families throughout the care planning process
  • Maintain accurate and up-to-date records of client interactions and progress
Requirements:

To be successful in this role, you will need:

  • A university degree in a related field, such as social work or nursing
  • Minimum three years of experience in direct care or case management
  • Excellent communication and interpersonal skills
  • Ability to work effectively in a team environment
What We Offer:

We offer a competitive salary range of $65,000 - $80,000 per year, depending on experience.
