About the Role:
We are seeking a skilled and compassionate Community Healthcare Specialist to join our team at ParaMed Home Health Care. This is a full-time opportunity for a Registered Nurse (RN) to make a meaningful difference in the lives of patients in their homes.
Job Summary:
The Community Healthcare Specialist will provide high-quality patient care in various settings, utilizing assessment tools and techniques to understand individualized needs. Key responsibilities include executing and continuously monitoring care plans, engaging patients through a collaborative approach, and maintaining accurate professional patient records.
About You:
To succeed in this role, you will possess registration with the CNO as a Registered Nurse in good standing, with experience in delivering community care being an asset. Reliable and flexible, you must be willing to travel to and from patient residences within a designated area and share on-call coverage to ensure patients receive support during their time of need.
