Job Title: Medical Care Specialist
About the Role:
We are seeking a compassionate and skilled Medical Care Specialist to join our team at Bayshore Healthcare. As a key member of our inter-professional team, you will provide specialized services in the management of patient care, working collaboratively with healthcare professionals to ensure timely responses to patient needs.
Responsibilities:
- Conduct patient assessments and develop comprehensive care plans.
- Order and interpret diagnostic tests, prescribe medications, and perform procedures within your legislated scope of practice.
- Participate in interdisciplinary team meetings to coordinate patient care and ensure effective communication.
- Provide direct patient care, focusing on health promotion and maximizing patient safety and function.
Qualifications:
- Completion of a recognized Primary Care or Adult Nurse Practitioner program with a minimum of 3 years of geriatric experience.
- Able to prescribe controlled substances as authorized by CNO.
- Master of Nursing.
- Current Extended Class certification with the College of Nurses of Ontario.
