Job Overview

We are seeking a skilled Gerontology Care Specialist to join our Home Health team in New Westminster, BC.

This is a casual opportunity for a dedicated professional who can provide high-quality care to clients and their families in the comfort of their own homes.

Key Responsibilities
  • Establish a strong therapeutic relationship with clients through effective communication and interpersonal skills.
  • Conduct thorough assessments, develop individualized care plans, and implement interventions to meet client needs.
  • Collaborate with interprofessional teams to ensure comprehensive client care.
  • Provide direct client care, identify additional support services required, and assign tasks to Community Health Workers as needed.
  • Facilitate regular care conferences to review client progress and adjust care plans accordingly.
Skills and Qualifications

To succeed in this role, you must have:

  • Strong knowledge of gerontology and adults living with complex frailty.
  • Ability to integrate and evaluate pertinent data from multiple sources.
  • Excellent communication and interpersonal skills.
  • Capacity to work independently and prioritize tasks effectively.

Salary Range: CAD $62,400 - CAD $83,200 per annum, based on experience.
