CarePartners is a leading home health care provider in Ontario, offering a range of services including nursing, personal support, therapy, and rehabilitation support. We are currently seeking a skilled Occupational Therapist to join our dynamic Rehabilitation Team.

Key Responsibilities:
  • Provide high-quality patient care through professional assessment, coordination, and supervision of Community Support Workers
  • Develop and maintain effective Care Plans in collaboration with Home and Community Care Support Services (HCCSS)
  • Provide guidance on workplace Health and Safety
  • Master's Degree in Occupational Therapy from a recognized university
  • Proven computer skills, including email, MS Word, and Outlook
  • Excellent English verbal and written communication skills
  • French language skills are an asset
