Job Summary

We are seeking a skilled Case Management Worker to join our team at Homes First. As a key member of our staff, you will play a vital role in supporting our clients and ensuring they receive the best possible care.

Key Responsibilities
  • Develop and implement individualized case plans to meet the unique needs of clients
  • Provide ongoing support and advocacy to clients, connecting them with community resources and services
  • Collaborate with internal and external stakeholders to ensure seamless service delivery
  • Conduct regular assessments and evaluations to monitor client progress and adjust case plans as needed
  • Maintain accurate and up-to-date records of client interactions and case plans
Requirements
  • High school diploma or equivalent required; post-secondary education in a related field an asset
  • Minimum 1 year of experience in a case management or related role
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Strong problem-solving and critical thinking skills
