We are seeking a skilled and compassionate Community Health Registered Nurse to join our team at Flemingdon Health Centre. As a key member of our Primary Care team, you will play a vital role in delivering high-quality primary health care services to our clients.
Responsibilities:- Provide comprehensive primary care to clients, including assessment, health promotion, and education.
- Coordinate client care with specialists and other healthcare agencies.
- Provide telephone advice and triage for requests, abnormal results, and health prevention issues.
- Initiate health teaching and health promotion with clients as part of a clinical assessment and groups.
- Participate in a home-bound program and/or do home visits as required.
- Registered Nurse registered in good standing with the College of Nurses of Ontario.
- Bachelor's Degree in nursing.
- Minimum of three years' experience as a registered nurse within a community health centre setting.
- Skills in assessment, health promotion, and education, as well as counseling and understanding of community development.
- Able to work as a member of an interdisciplinary healthcare team.
- A dynamic and supportive work environment.
