Wellness Nurse

Sunrise Senior Living is seeking a dedicated and compassionate Wellness Nurse to join our team. As a key member of our healthcare team, you will play a vital role in promoting the overall well-being of our residents.
Responsibilities- Conduct regular wellness visits for all residents to identify their ongoing needs and services.
- Collaborate with the Resident Care Director to evaluate and document resident health care services and status.
- Monitor and follow up with residents who have experienced falls, accidents, or incidents, documenting and reporting as required.
- Communicate effectively with residents' families and responsible parties to ensure their medical needs are met.
- Assist in maintaining accurate and up-to-date resident wellness files.
- Provide clinical support and assistance to community team members as needed.
- Graduate of an approved college or school of nursing.
- Current state license as a professional Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN).
- Minimum of one year experience in home health, assisted living, hospital, or long-term care environment.
- Excellent communication and interpersonal skills.
- Ability to work weekends, evenings, and flexible hours.
