Wellness Nurse

Sunrise Senior Living is seeking a dedicated and compassionate Wellness Nurse to join our team. As a Wellness Nurse, you will play a vital role in ensuring the overall well-being of our residents, providing high-quality care and services that promote their physical, emotional, and social well-being.
Responsibilities- Conduct regular wellness visits for all residents to identify their ongoing needs and services.
- Assist the Resident Care Director in conducting resident evaluations and gathering data to ensure the highest quality resident services.
- Monitor and follow up with residents who have experienced any falls, accidents, or incidents, documenting and reporting as required.
- Document all pertinent information in the resident wellness file and maintain effective communication with residents' families and responsible parties.
- Collaborate with the Resident Care Director and community team to promote infection control standards and ensure compliance with universal care precautions.
- Regularly monitor each resident's medication and treatment profile to ensure accurate administration and documentation.
- Participate in Sunrise Quality Assurance Meetings and Clinical Meetings to ensure quality care and services.
- Graduate of an approved college or school of nursing.
- Current state license as a professional Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN).
- Minimum of one year experience in home health, assisted living, hospital, or long-term care environment.
- Knowledge of nursing practices, techniques, and methods applied to health and wellness resident care, and federal, state, and local regulations.
- Ability to handle multiple priorities, possess written and verbal skills for effective communication, and demonstrate good judgment, problem-solving, and decision-making skills.
