Job Title: Clinical Health Systems Specialist

Job Summary:

Under the general supervision of Pharmacy Manager, the Clinical Health Systems Specialist will be committed to quality improvement processes, mentorship, education, and critical decision-making regarding medication management. The pharmacist is actively involved on interprofessional teams to provide exceptional patient care.

Key Responsibilities:

• Develop and implement quality improvement initiatives to enhance patient care and safety
• Mentor and educate healthcare professionals on evidence-based practices and medication management
• Collaborate with interprofessional teams to provide comprehensive patient care

Requirements:

• Doctor of Pharmacy or Bachelor of Science in Pharmacy with Accredited Canadian Pharmacy Residency (ACPR)
• Registered Pharmacist Part A with Ontario College of Pharmacists
• Excellent interpersonal and interprofessional teamwork skills
• Strong problem-solving and critical thinking skills

Assets:

• Previous oncology pharmacist experience
