Clinical Pharmacist Specialist-FT

**Job Summary:**

**Key Responsibilities:**

* Provide medication management expertise to patients and healthcare teams

**Requirements:**

* Doctor of Pharmacy or Bachelor of Science in Pharmacy with relevant experience
* Registration with the Ontario College of Pharmacists
