Nurse Practitioner, Glomerulonephritis Specialist

About the Role
We are seeking a highly skilled and experienced Nurse Practitioner to join our Glomerulonephritis Clinic team. As a key member of our multidisciplinary team, you will provide comprehensive patient care to individuals with chronic kidney disease, working in collaboration with nephrologists, primary care providers, and other healthcare professionals.
Key Responsibilities
- Provide advanced nursing care to patients with glomerulonephritis, including health assessment, diagnosis, and management of chronic kidney disease.
- Develop and implement individualized care plans, working in partnership with patients, families, and healthcare teams.
- Collaborate with nephrologists, primary care providers, and other healthcare professionals to ensure seamless transitions and comprehensive care.
- Participate in quality improvement initiatives, education, and research activities to advance patient care outcomes.
- Act as a resource and mentor to other healthcare professionals, promoting best practices and evidence-based care.
Requirements
- Current registration in the Extended Class with the College of Nurses of Ontario.
- Masters Degree in Nursing.
- Nurse Practitioner Certificate.
- 1-2 years of demonstrated experience in acute medicine, with a focus on nephrology.
- Excellent communication and interpersonal skills, with the ability to work collaboratively with healthcare teams.
- Strong analytical, problem-solving, and critical thinking skills.
- Ability to work independently and as part of a team.
