Job Title: Nurse Practitioner - Nephrology Specialist

Job Summary:

The University Health Network is seeking a highly skilled Nurse Practitioner to join our Nephrology team. As a Nurse Practitioner in Nephrology, you will be responsible for providing high-quality patient care, conducting assessments, and developing treatment plans in collaboration with the interdisciplinary healthcare team.

Key Responsibilities:

  • Conduct comprehensive assessments of patients with renal failure requiring in-center hemodialysis
  • Develop and implement individualized treatment plans in collaboration with the healthcare team
  • Provide education and support to patients and their families
  • Collaborate with the Nephrology interdisciplinary healthcare team to ensure seamless care
  • Participate in quality improvement initiatives to enhance patient outcomes

Requirements:

  • Master's degree in Nursing
  • Registered Nurse (EC) certification
  • Nurse Practitioner (NP) Adult or Primary Care certificate
  • Registered with the College of Nurses of Ontario in good standing
  • Three to five years clinical practice in Nephrology nursing in an acute treatment center
  • National certification in Nephrology Nursing from the Canadian Nurses Association
