Role Summary

Humber River Health is seeking a skilled Nurse Practitioner Stroke Prevention Clinic Specialist to join our team. As a key member of our interdisciplinary team, you will provide assessment, coordination, and provision of services to clients and families in our Stroke Prevention Clinic.

Key Responsibilities

  1. Develop and evaluate the Stroke Prevention clinic, ensuring high-quality patient care and adherence to best practices.
  2. Responsible for day-to-day operations, making service delivery and program changes as needed.
  3. Promote stroke prevention best practices in the care of clients with suspected transient ischemic attack (TIA), minor non-disabling stroke, and/or vascular risk factors.
  4. Triage referrals using professional judgment, coordinating diagnostic and laboratory testing within the RN(EC) scope of practice.
  5. Provide pharmacological interventions, including prescribing medications as per RN(EC) scope of practice and medical directives.
  6. Initiate, renew, titrate, and discontinue pharmacotherapy as appropriate, ensuring vascular risk factors for stroke are well managed, meeting stroke prevention guideline targets.

Requirements

  • Master of Nursing Degree
  • NP – Adult, RN(EC), Registration in good standing with the College of Nurses of Ontario
  • Member of the Registered Nurses Association of Ontario preferred
  • Minimum 3 years experience in geriatrics and/or community nursing services
