Nurse Practitioner, Glomerulonephritis Specialist

Humber River Health is seeking a highly skilled Nurse Practitioner to join our Nephrology Program. As a specialist in glomerulonephritis, you will be responsible for providing comprehensive patient care and working collaboratively with our multidisciplinary team to deliver high-quality care.

Key Responsibilities:

  • Provide specialty-based focus and advanced assessment skills to formulate clinical decisions and provide comprehensive patient care for clients with CKD
  • Facilitate interdisciplinary care by participating in regular team meetings and providing a linkage between the client, members of the interdisciplinary team, physicians and other members of the health care team
  • Liaise with community-based providers, advocating on behalf of clients and families and participation in patient and family conferences as needed

Requirements:

  • Current registration in the Extended Class with the College of Nurses of Ontario required
  • Masters Degree in Nursing required
  • Nurse Practitioner Certificate required
  • 1-2 years of demonstrated experience in acute medicine
  • Nephrology experience would be an asset
