Job Summary

As a Care Transition Specialist with William Osler Health System, you will play a key role in ensuring seamless transitions for patients throughout their healthcare journey. This dynamic role requires a strong understanding of healthcare systems and a passion for delivering exceptional patient care.

Main Responsibilities

Collaborate with inter-professional teams to develop and implement effective discharge plans that meet the unique needs of each patient.

Conduct risk assessments and identify potential barriers to discharge, working closely with the healthcare team to develop strategies for timely and safe transitions.

Communicate regularly with patients and families to ensure their needs are met and they are supported throughout the transition process.

Requirements

A bachelor's degree in a healthcare-related field and relevant experience in case management or utilization review are essential for this role.

Maintaining accurate records and statistical information is critical to identifying areas for improvement and optimizing patient care.

This role requires strong leadership skills, with the ability to motivate and empower inter-professional teams to deliver high-quality patient care.
