Primary Care Nurse Practitioner

We are seeking a highly skilled and enthusiastic Nurse Practitioner to join our innovative team at Rhynocare. As a key member of our interprofessional team, you will provide exceptional care to our geriatric patient population through our virtual care platform.
Key Responsibilities:- Conduct comprehensive health assessments and develop evidence-based care plans
- Provide technical skill and adaptability in utilizing virtual care software
- Offer clinical care through our virtual hub and obtain consent for treatment
- Diagnose illness and perform interventions within the NP scope of practice
- Order and interpret diagnostic tests, prescribe medications, and manage disease-specific pain and symptoms
- Follow up with clients and provide education, coaching, and counseling related to acute and chronic conditions
- Participate in regular business meetings to contribute to program development and ongoing monitoring and evaluation
- Be available for on-call and after-hours services for Rhynocare clients
- Lead by example in critical thinking, problem-solving, ethical decision-making, and evidence-based practice
- Completion of a recognized Nurse Practitioner program
- Primary Health Care Nurse Practitioner preferred
- Current registration with the College of Nurses of Ontario in the Extended Class
- Minimum 3 years nursing experience, including 2 years experience as an NP
- BLS-HCP certification
- Ability to speak a second language is an asset
- Access to a home office
