Care Coordinator for Patients with Gastrointestinal Issues

We offer a competitive salary of CAD $37.05 per hour.
Job Description:
This role involves working collaboratively with interdisciplinary care teams to provide direct patient care activities such as assessing, planning, implementing, and evaluating interventions. You will also participate in various meetings, including care conferences, and assist with the orientation of new staff and students as required.
Responsibilities:
- Assess patients' conditions and develop plans to address their healthcare needs
- Collaborate with healthcare professionals to coordinate care and implement treatment plans
- Communicate effectively with patients, families, and healthcare teams to ensure quality care
- Participate in care conferences and contribute to decision-making processes
Qualifications:
To be successful in this role, you must have:
- Current practicing registration as a Licensed Practical Nurse with the British Columbia College of Nurses and Midwives (BCCNM)
- Completion of recognized post-basic education in gastroenterology/GI endoscopy nursing or an equivalent combination of education, training, and experience
Skills and Qualities:
- Demonstrated knowledge of pathophysiology related to common diagnoses specific to the designated patient group
- Demonstrated skill in performing LPN scope of practice and standards of care relevant to the designated patient population
- General knowledge of Trauma Informed Practice, Harm Reduction approach, and Strengths-based care
- Ability to communicate effectively, both verbally and in writing
