Community Health Nurse Specialist

This position involves working with patients of all ages, providing complex clinical care and helping them remain comfortable in their own homes.
About the RoleAs a Registered Practical Nurse (RPN) with ParaMed Home Health Care, you will be part of a dynamic team that prioritizes patient-centered care. This is a full-time, temporary contract for six months, with a biweekly schedule of 84 hours including alternating weekends and travel between our Guelph and Cambridge clinics.
Key Responsibilities- Participate in scheduled home visits to assess patient needs and provide appropriate care within your scope of practice
- Deliver safe and ethical nursing care, utilizing assessment tools and techniques to identify individualized needs
- Implement and continuously monitor care plans, making adjustments as necessary to ensure optimal outcomes
- Maintain and update professional patient records related to encounters and care plans
- Registration with the CNO as a Registered Practical Nurse in good standing
- Valid driver's license and access to a vehicle
- Experience in community care delivery is an asset
- Reliability and flexibility are essential for this role
- Willingness to travel to patient residences within a designated area
- Experience working with vulnerable populations is beneficial
- Prior experience with on-call coverage, including after-hours shifts, is required
We offer an estimated salary range of $60,000 to $80,000 per annum, based on location and experience.
