About the Role

CarePartners is seeking a skilled Occupational Therapist to join our dynamic Rehabilitation Team. As a key member of our team, you will play a critical role in providing high-quality patient care and contributing to the growth of our therapy/rehabilitation program.

Key Responsibilities
  • Provide professional assessment and treatment of patients in their homes and communities
  • Develop and maintain effective care plans in collaboration with the care team
  • Coordinate and supervise Community Support Workers in the field
  • Work independently and as an integral member of an interdisciplinary care team
  • Provide guidance on workplace Health and Safety
Requirements
  • Master's Degree in Occupational Therapy from a recognized university
  • Registration with the College of Occupational Therapists of Ontario
  • Proven computer skills and strong assessment skills
  • Excellent English verbal and written communication skills
  • Reliable vehicle for regular travel
