Patient Care Coordinator

About the Role:
The Patient Care Coordinator, also known as a Health Optimization Specialist, plays a vital role in supporting clients/residents/patients to achieve and maintain their optimum level of health within a people-centered care approach.
Working to the full scope of practice, this dynamic individual will engage in ongoing education and maintain competency in acute, transitional, personal care home, primary health care, public health-healthy living, and home care settings.
Key Responsibilities:
- Apply nursing process to support clients/residents/patients in achieving and maintaining their optimum level of health
- Engage in ongoing education and maintain competency in acute, transitional, personal care home, primary health care, public health-healthy living, and home care settings
- Exercise initiative and independent judgment in determining work priorities and methods
Qualifications:
- Current active registration with the College of Licensed Practical Nurses of Manitoba (CLPNM)
- Current Basic Life Support (BLS) certification
- Knowledge of Professional Standards of Practice and Competency Profile for Licensed Practical Nurses in Manitoba
- Strong communication, interpersonal, and problem-solving skills
- Ability to work independently and as part of a team
- Flexible and adaptable approach to work
Compensation and Benefits:
We offer a competitive salary of approximately $ $83,000 per year, based on experience and qualifications.
Conditions of Employment:
- Completes and maintains a satisfactory Criminal Record Check, Vulnerable Sector Search, Adult Abuse Registry Check, and Child Abuse Registry Check
- Current immunization as per Southern Health-Santé Sud policy
