About the Role

We are seeking an experienced Licensed Practical Nurse (LPN) to join our team as a Healthcare Nurse at our Sunrise Senior Living community in Erin Mills. In this role, you will be responsible for providing high-quality nursing care to our residents, working closely with our Resident Care Director and other healthcare professionals to ensure seamless delivery of services.

Responsibilities
  • Assess resident needs and develop individualized care plans to promote optimal health and wellness.
  • Provide direct nursing care to residents, including administering medications, performing vital signs, and documenting care.
  • Collaborate with interdisciplinary teams to coordinate resident care, including medical treatments, therapies, and social activities.
  • Maintain accurate and detailed records of resident care, including medication administration, vital signs, and progress notes.
  • Communicate effectively with residents, family members, and healthcare providers to ensure timely and effective care.
  • Participate in quality improvement initiatives to enhance resident care and satisfaction.
Qualifications
  • Graduate of approved college or school of nursing with current state license as a LPN/LVN.
  • Minimum one year experience in home health, assisted living, hospital, or long-term care environment.
  • Demonstrated knowledge of nursing practices, techniques, and methods applied to health and wellness resident care.
  • Ability to handle multiple priorities and work independently with minimal supervision.
  • Possesses written and verbal skills for effective communication and ability to teach and guide team members.
