About the Role:
We are seeking a highly skilled and experienced Forensic Community and Liaison Social Worker to join our team at BC Mental Health and Substance Use Services. As a key member of our team, you will work closely with clients, families, and healthcare professionals to provide comprehensive social work services in a forensic mental health setting.
Key Responsibilities:
- Conduct thorough assessments and develop individualized treatment plans to address clients' complex needs.
- Provide case coordination and advocacy services to ensure seamless transitions between healthcare providers and community resources.
- Maintain accurate and concise documentation of client information, adhering to regulatory requirements and professional standards.
- Collaborate with multidisciplinary teams to provide clinical services, education, and support to clients and their families.
- Facilitate access to psychiatric and social services through referrals, site visits, and home visits, as needed.
- Participate in pre-release planning conferences and consult with clinical teams to determine client needs and develop strategies for community reintegration.
Requirements:
- Bachelor's degree in Social Work from an accredited institution.
- Two years of recent experience working in a mental health setting, with a focus on forensic social work.
- Full registration with the British Columbia College of Social Workers (BCCSW) is required.
- Valid BC Driver's License and access to a reliable vehicle for local travel.
- Current Cardiopulmonary Resuscitation (CPR) Certificate.
