Job Title: Wound Care Specialist

Job Summary:

Closing the Gap Healthcare Group is seeking a highly skilled Wound Care Specialist to join our team. As a NSWOC Nurse, you will play a critical role in providing high-quality care to our clients with wound, ostomy, and continence needs.

Key Responsibilities:

  • Assess and manage clients with wound, ostomy, and continence issues, providing evidence-based care and education to promote optimal outcomes.
  • Collaborate with the healthcare team to develop and implement comprehensive care plans, ensuring seamless transitions and continuity of care.
  • Provide clinical advocacy and support to clients and their families, addressing their unique needs and concerns.
  • Conduct thorough assessments, including virtual consults, to identify client needs and develop effective care plans.
  • Stay up-to-date with the latest wound care management techniques, best practices, and research, applying this knowledge to improve client outcomes.

Requirements:

  • Current certification as a Registered Nurse (RN) with the CNO.
  • Completion of a wound, ostomy, and continence program (NSWOC) or equivalent experience.
  • Minimum of two years' experience in community or clinical/hospital settings, with a focus on wound care.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with clients, families, and healthcare teams.
  • Ability to work independently and as part of a team, prioritizing tasks and managing time effectively.
  • Current certification in Standard First Aid & CPR/AED (Level C) and Mask Fit test.
