Closing the Gap Healthcare Group is dedicated to delivering exceptional care to Canadians in their homes and communities.
We are seeking a skilled NSWOC Nurse to join our team. Our ideal candidate will have a passion for providing high-quality care to clients with wound, ostomy, and continence needs.
Key Responsibilities:
- Assess and manage clients with wound, ostomy, and continence care needs
- Develop and implement individualized care plans to promote optimal client outcomes
- Collaborate with healthcare teams to ensure seamless care delivery
- Provide education and support to clients and their families
- Conduct assessments for virtual consults
Requirements:
- Current certification as a Registered Nurse (RN) with the CNO
- Successful completion of a wound, ostomy, and continence program (NSWOC) or WOCN program
- Minimum of two years' experience in community or clinical/hospital setting
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
